Authorization for Administration of Medication Form

Parents, if your asthmatic child attends a Clark County school, this is an official form, authorizing the school to administer both allergy medications and asthma medications to your student, while they are in the school’s care. Please download the Asthma/Allergy Medication Administration Form, fill it out, and hand it to the school’s resident medical professional.
